I had no issues with my install and don't really understand what you are saying is wrong on yours, do you have pictures? It almost sounds like the orientation might be off and once you realign it all it should work out.

So, I called Mishimoto and they had a quick suggestion, "try mounting the tube with the flange inside the heat shield." To do this, I had to remove the rubber strip from the hole in the heat shield, place tube through the hole, then reinstall the rubber strip (absolute PITA). The silicone intake pipe has just enough length to still clamp down onto the MAF tube, and the filter is clamped on solid. I'll give another update after couple days if this solves the issue of filters falling off.
