It's the sole primarily. As you mentioned the sole is flat, but the heel is rounded in order to make heel toe downshifting easier and the soles come up the sides more than on most other shoes in order to provide more grip on the pedals when using the sides of your foot. These are all subtle things, but they add up to make three pedal driving easier.
Keep in mind that these are not racing shoes like a race car driver uses, but driving shoes to enhance the driving experience, especially for manal transmission cars.
