Hard to tell from the pics.. the first one is kinda blurry and the second one there appears to be pits and little metal chunks.

Are there pits in the crown and/or the head chamber?

It not pits,it's little drops of antifreeze. It's the back cylinder so it's almost impossible to take a good picture, but if you look close there are darker vertical streaks on the cylinder wall.

On my own setup, at 8-9 psi I see 78-80 lbs/min at similar rpm and ambient temps... so I would have *GUESSED* (because I dont really know) that lbs/min should be proportionately higher. Looking for more experienced input there... I dont have any context to know what airflow lbs/min should look like at 11 psi vs 8 or 9 psi.
What you're saying (MAF cal off) makes a lot of sense. If there is more air going in than the ECM knows about, then of course it will be leaner than intended. Commanding .81 and getting .87 sure looks like a calibration problem.

Thanks for the pics. Would love to see the piston when it comes out too. Ive seen the log and seeing the associated damage helps to put the bigger picture together for me.

Pics are awesome and thanks for the clarification. It's hard to get to those back cylinders.

Pull the piston and maybe you can get a better pic.

On both I see the bore marks, but the first one is blurry, and it looks like that area on the piston crown by those bore marks is pocked. Hard to make out what's up in the second pic (piston wise).

On the other side of the coin, if you did lean out, that would/could cause seizing.

The skirt and crown to the first ring will have more clues as to what happened too.
