I think it's by design. I'll let the auto people answer.

I've noticed this as well in the M6. If you're coasting to a stop in neutral and you try to rev nothing happens. But once you stop you can rev.

Never noticed because I don't do it. Back in the day, unsure if true or not but you could cause excessive wear on bearings and rings due to piston slap. A piston not under load and freewheeling is not good for the rotating assembly. Unsure if this is true now with better engine tolerances.

Yeah the electronic throttle throws you off a bit - you have to completely floor it to get it to rev in that case, but that's excessive. Because of this I always use rev-match. I've tried running without it but the inability to know when the throttle will or won't respond how you expect makes it very tough to downshift smoothly.

I don't know why anyone would throw it in N while driving and rev it, so I can't speak to that, but I have revved it in park to try to isolate a noise, and it revs slowly unless you give it half throttle or more. But it revs. I'd imagine it's a safety feature to keep people from bouncing off the limiter on accident when reaching for luggage in the back seat or something.
