You haven't explained what "lugging" actually *is*, yet here you are asserting a scenario where "it" is happening. If you can address any of those bullet points, we'll be heading in the right direction.
For your last question, its called Mechanical advantage. Your Transmission does that as I explained above. To better understand get a 20lb dumbell and hold it to your chest with one hand, now extend your hand straight hand out, you still holding it straight at the same level? Likely not. You can hold a 20lb dumbbell with it being close to you (Advantage) but, when held out you are at a disadvantage at holding it(due to the moment arm). Did the weight get heavier? Did you get weaker?

So while an engine can produce X amount of power on a dyno at what ever RPM that doesn't explain what happens when the dumbbell is extended out or in a cars case High loads at low RPMs. Hopefully this explains it.
