You definitely need a new belt. There is nothing else I can say.

There is no way you drove the car for a month with the belt like that! IMHO

Those belts just don't slip off with normal driving or even extreme situations. A road hazard could have caused it. I would bet there's an idler that is failing somewhere. Rotate all the idlers by hand and make sure all of them are free.

I'm just as hesitant as anyone else about taking my car to the dealership for work, but this is something I would run by them. I'd want to make sure that they're aware of what happened, that it's noted in the service history, and that they give the car a good inspection to make sure it's all ok and get that belt replaced.

Yes, warranty should still be good. I'm so skeptical about bring my car to a Chevy dealership. Would this be a one day fix and inspection or are they going to try to tell me to come back in a few days? I'd much rather just sit there and watch them work if at all possible...

is the supercharger hard to turn?
if the bearings are going out it could pop the belt.

Find someone in your area that speaks well of a service department, there is something wrong, like that say maybe a pulley, it could cost you more in the long run. a belt doesn't come off unless something out of alignment or something got in the way.And if its something worse, when you need it fixed because the price out of pocket is great, the dealer will say it's your tab !
