Just in case anyone ever runs into this since no one had much info to share. Hooked the data reader up to the car checked the volt readings of all 4 sensors. 3 were reading up to 0.9 volts and bank 1 (driver side front) was reading 0 to 0.005 volts. Pulled sensor replaced, cleared code, and rechecked voltage now reading up to 0.9 like its supposed to. Car only has low mileage only thing I can think caused failure was I got dealt the short straw from GM.

How is a failed sensor GMs fault if you changed to headers?
Why is your O2 voltage reading so high? .9 indicates that the system is rich is this all the time, only cold, while driving, was the sensor black with soot or white to grayish? I ask because you may have fixed the failure but not the cause of the failure |

I believe he was saying that it would fluctuate up to a max of 0.9 not steady state at 0.9. I wouldn't stress about why it failed. It could have gotten stressed (wire pulled, dropped, etc) somehow during the swap and finally gave out.

Just to confirm, up until now, your catless cooks didnt throw any codes? I want to get them but I don't want to have to tune the car and void the warranty.

Headers will need a tune no matter what, catless will also require a tune no matter what as the rear O2 sensor codes will have to be turned off. Most headers move the front O2 sensor location further from the engine and can affect how long it takes the sensor to "light off" which will cause a P01xx code of some sort. The rear O2 sensors only tell the ECM if the cats are functioning, so when you go catless those rear O2 will trip a code as well and will require the rear O2 codes to be tuned out. |

Yea I am catless and no there where no codes because the car has been tuned correctly to avoid them. Code came on because the sensor failed and that code is not disabled during tuning for that very reason if failure occurs.

Just to add to the thread and update have drove the car about 400 miles since changing the sensor. No issues data logs look great.
