I attached 3 images: first shows the filter falling off, second shows where the flange used to be outside the air box, and third image shows where the flange is now with filter up tight up against it.
The MAF tube has a flange that presses up against the outside of the heat shield in a normal install to make installation easy (not spending a ton of time positioning the MAF tube). However, with the flange as tight against the heat shield as possible, the tube doesn't stick into the air box very far. This was causing my filter clamp to slowly pull the filter off when tightened even if the engine wasn't running. I could drive fine to work and find the filter had fallen off by lunch.
So I called Mishimoto and they suggested shoving the flange through the heat shield hole into the air box. Intake tube can still clamp to MAF tube and the filter hasn't fallen off so far thanks to extra clamp surface area. Keeping fingers crossed, don't like the idea of losing my filter during my 50 mile one way commute to work...
