You need to and should not omit it to slightly adjust ride height. Remember suspension bushings are solid molded rubber between an inner and outer sleeve. Any suspension arm movement twists the outer sleeve with respect to the inner. Correctly indexed movement would be ~x degrees of twist in both the up and down directions. Incorrectly indexed could be as much as 0 twist at full down travel and 2x twist at full up travel resulting in additional stress and premature bushing failure. Suspension bushings shouldn't be working as helper springs.
