There is actually an internal GM service bulletin regarding this exact issue, and their diagnosis of the issue cause is that if there is too much tension on the headlight wire harness, then water can drip down the line into the connecter causing a short and preventing the fan to come on. I mentioned this when I took my car in and they were able to confirm this bulletin. I had 2 codes. P0480 and P0691 both referring to the cooling fan problem. They had to replace the cooling fan module.
