Well this is coming out about how I usually see people respond. 4/5 people carry with one in the chamber. I wanted to stay neutral until some people responded, but my opinion on the subject has been expressed by many here. When in an emergency situation (which is the only time a concealed weapon should be out other than being put away at night) you don't have the time that movies portray to rack the slide of a semi auto. Obviously revolvers always have one "in the chamber" otherwise it is unloaded and therefore worthless. I feel more comfortable with an external safety, but I have carried in the past with something like a Glock that doesn't have an external safety other than the trigger safety.
The important thing here is that if you are going to carry or even own a weapon, you need to become educated in proper handling and shooting techniques. Once you become educated practice, practice, practice! Go shooting on a regular basis so if/when you might ever need to draw a gun you are able to operate the gun safely.
