I dont remember what it was in season 1 (watched when it first came out) that irritated me enough to stop watching, but theres other things that I feel is a rather large unnecessary change. In the TV Show, Robb's wife was changed...probably more for the shock effect during the Red Wedding. Since the books have his wife not at the wedding (and not pregnant), she essentially disappears. Frankly, doesn't really change anything, and for the TV Show, I think it worked well. Talisa wasn't working for the Lannisters, and I'm not sure where you got that idea.
As far as Ramsay Snow, there were changes from the book, however he did take Winterfell from Theon in the Great Sack of Winterfell. How he insinuated himself into the ranks beforehand is different and not shown in the TV show.
