Didn't happen to mine since I'm not completely finished with the install yet. But it did happen to a friend of mine, and also to another guy I know.
Dave mentioned it could be from running too high pressure or powering up the system before uploading his software. But according to my friend, none of that applied, everything was running fine until around 2,000 miles in, then it suddenly lost fuel pressure.
When he checked, the cover plate was visibly warped and lifted on the outer edge, and the O-ring had blown out. He got a new, updated cover plate that's supposedly made stronger.
Dave said this kind of failure is very uncommon, but I just want to be on the safe side and install the updated plate now, especially while not everything is back together yet and it's still easier to drop the tank again.
