These wonderful folks sell an all-inclusive prep kit for the LT2 IM for $45. It does at the time of this posting show they are out of stock, but you can see what's included and pick most of those parts up yourself. The MAP sensor harness extender is a little short for how I'd like to see it routed. At least mine came a little short. If you really know your parts you can pick up the connectors and do your own wiring to extend the harness yourself.

Don't forget to shave the bottom on the manifold.

If you're trying not to modify that brake booster line, I think you're just going to struggle. I used a heat gun and pulled the line of the IM side of the check valve and replaced it with the supplied hose and brass barb fitting. I also have my vacuum block for my BOV tapped inline here as well. I felt there was enough going on at the front of the IM I didn't want that vacuum block muddying up the works just hanging out.
