It is more rare to have a failure like that but it can still happen.
Autos in case of shifting failure can engage wrong gears or several at the same time and lock up. In a manual that's way harder to do.

I can't think of a single time I have seen or heard of a Tremec manual coming apart.
I have heard of broken shift forks and gears breaking, but nothing this serious.
Has someone seen a tremec blow up like this? If so, send me a link. Just curious...
