I don't think I've seen that, or this with an automatic or dual-clutch either. This sort of incident on a dyno has always been motor failure as far as I recall... I've seen a lot of those videos.
It can happen if the case breaks. Normally wou will wipe the teeth or cut clean one of the shafts.
It is more rare to have a failure like that but it can still happen.

Autos in case of shifting failure can engage wrong gears or several at the same time and lock up. In a manual that's way harder to do.
