I find the semi-metallics to be very hard on the rotors. I have ceramics up front and the semis in the rear. there is more scoring and, yes, more brake dust in the rear.
The deep gouge could be foreign material stuck to the pad, as arpad suggests. the question is, if you bother to check, do you want to just go ahead with new pads and rotors while you have it apart?

I put on same pads as OE, which I believe are semi-metallic
