This piece looks very professional, the fit and finish is 100% OEM grade, and it's a perfect match with the accelerator pedal cover. If Chevy sold this with the sport pedal accessory kit, I don't think anybody would complain.
I snapped off the dead pedal and used thin, super sticky underlayment tape to attach the cover, which didn't require any curing time and appears to work fine (much better than super glue), we'll see how long it lasts.

I used super glue, which has worked fine. Sadly I only had 1 test piece so couldn't attempt other methods.
In hindsight, silicon adhesive probably would have been easier for me. Arpad also just reported success with double sided tape.
So my vote is silicon adhesive or double sided tape.
